Improved pam-session patch

I have moved the pam session calls to the sshd.c before the monitor
forks the child. I also removed various pam calls from session.c
because they would be duplicate to the calls in sshd.c.

I tested this here in various root/non-root privsep/no-privsep
combinations and it seems to work fine, pam_ calls are done as uid 0
and they are always called.
