Correct the sftp syntax to match actual behaviour

The second and third usage forms in the sftp(1) man page incorrectly
indicate that a targe hostname is optional. An extraneous pair of
square brackets should be removed.

The attached patch fixes the syntax in the man page and in the sftp
usage output.
