I vote for SSH_ASKPASS="always:/path/to/file". May be modification code
has to be similar to that I propose in #9.

For the current code in a X-terminal "nohup ssh-add .."  (DISPLAY is
set) is enough to get SSH_ASKPASS window.

May be Jim and Damien will update respective ssh-askpass GUI programs
to use tty if DISPLAY is not set ?
