PermitRemoteOpen directive

This is an implementation of PermitRemoteOpen, including regress tests
and a small refactoring of the permitopen permissions to enable more
sharing of code.

TODO: authorized_keys permitremoteopen, PermitRemoteOpen="123" (i.e.
bare port number), manual pages.
