using a recent version of Redhat Linux, the config file in
packages/redhat/sshd.pam should be more useful.

If you aren't using this file, then you should be.
If you already have, then the problem lies with your PAM - RH6.1 has an update for PAM.

FYI, i'm using a a stock installation of RH6.1 + all the updates.
