sshd refusing logins from certain hosts

Ok this is a weird one.

I cannot ssh into my server from the ISP directly.

I CAN ssh into my server from the VPN to my company from the ISP.

I can ssh into my server on a different port from the ISP directly and the VPN.

I am using PAM authentication.

I am using PuTTY

When it fails, it first asks me for my username, then says "connection closed".

any ideas?
