[PATCH] Requiring multiple auth mechanisms (updated)

This patch looked interesting but Mr. Ogata's seemed better suited for use with the 
out-of-band (obc) authentication method 
(https://bugzilla.mindrot.org/show_bug.cgi?id=1438) I was working on. I'm a novice, 
so I could easily be wrong.
