ssh_exchange_identification: Connection closed by remote host

In my experience this error is typically really a network problem. Try
doing manual telnet to port 22 on the server. This should show you the
server ssh version (i.e. the initial identification) like
"SSH-2.0-OpenSSH_4.3p2 Debian-7ubuntu1"

If it does not then there is your problem.
