[patch] spelling cleanup

Hi,
I have a series of spelling fixes [1] also available as a patch [2]. I'm
fairly comfortable that these changes should apply to current CVS head. If
you don't want some portion of the changes, please feel free to identify
the words, files, or directories to exclude.

You're also welcome to squash the changes.

It's of course possible to split this into things which will affect output
and things which won't if that's necessary, but I probably won't have time
for that for a while.
