Error with USE_POSIX_THREADS and OpenSSH-3.8p1

The attached patch is what I'm thinking of.  (It would also fix bug #445).

USE_POSIX_THREAD will probably go away as soon as there is a better fix 
for the krb/PAM issue, see:
